Initial Comments:
An offsite follow-up survey conducted on January 10, 2022 found that Brightstar of Lehigh Valley had corrected the deficiencies cited under the requirements of 28 Pa. Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of an onsite state relicensure survey completed on November 15, 2021.
